Billboard Proof of Performance Report

Thank you for helping us Pass It On! We appreciate your support of our messages by placing these billboards throughout your communities. Once the following proof-of-performance report is submitted and verified, we will pay your company a ONE TIME posting fee of $200 per unit for 14'x48' vinyl billboards, $50 per unit for 30-sheets and $25 per unit for Bus Shelter Display / Mall Kiosk sizes.

IMPORTANT: We need the following two documents to process your payment:

1. A completed W9. If you have one on file with us, we can use that, otherwise, please download a copy, complete it and fax it to us at 866-927-0442.
2. A copy of your invoice for these billboards. If we don't receive an invoice from you, we cannot pay you. Please include your invoice below, or fax it to us at 866-927-0442.
